iPASS Card Notification of Personal Information Protection

一卡通票證股份有限公司(以下稱本公司)依據個人資料保護法第八條第一項規定,向 台端告知下列事項,請台端詳閱:

一、 蒐集之目的:
0四0:行銷業務。
0五九:金融服務業依法令規定及金融監理需要,所為之蒐集處理及利用。
0六0:金融爭議處理。
0六三:非公務機關依法定義務所進行個人資料之蒐集處理及利用。
0六七:信用卡、現金卡、轉帳卡或電子票證業務。
0六九:契約、類似契約或其他法律關係事務。
0九0:消費者、客戶管理與服務。
一四八:網路購物及其他電子商務服務。
一八一:其他經營合於營業登記項目或組織章程所訂之業務。
一八二:其他諮詢與顧問行銷服務。

二、 蒐集之個人資料類別:
姓名、身分證統一編號、住址、住家電話號碼、行動電話、出生年月日、相片、電子郵遞地址及其他台端與本公司相關業務申請書及契約書填載內容,並以本公司與台端往來之相關業務或服務及自台端或第三人(例如:合作發行機構)所實際蒐集之個人資料為準。

三、 個人資料利用之期間、對象、地區、方式:
(一) 期間:以下所列期限最長者為準
1. 特定目的存續期間。
2. 相關法令規定之保存期間(如電子票證發行管理條例等)。
3. 因執行業務所必須之保存期間或契約約定之保存年限。
(二) 地區:以下列之利用對象之所在地。
(三) 對象:
1. 本公司、業務委外機構、與本公司合作發行之機構或有業務往來之機構。
2. 金融監理機關或依法有調查權或監理機關。
(四) 方式:合於法令規定以自動化機器或其他非自動化之利用方式。

四、 依據個資法第三條規定,台端就本公司保有 台端之個人資料得行使之權利及方式:
(一) 台端得向本公司查詢、請求閱覽或請求製給複製本,而本公司依法得視情形收取必要之成本費用。
(二) 台端得向本公司請求補充、更正個人資料,惟依法應提出適當之釋明文件。
(三) 台端得向本公司請求停止蒐集、處理或利用及請求刪除個人資料,但依法本公司因執行業務所必須時,得予以拒絕。
(四) 行使權利之方式:書面。

五、 台端不提供個人資料所致權益之影響:
台端得自由選擇是否提供相關個人資料,惟台端若拒絕提供相關個人資料時,本公司將可能無法進行必要之審核及處理作業,致無法提供台端相關服務或較佳之服務。

六、權利行使及告知事項之諮詢:
台端如欲行使上述個資法第三條規定之各項權利,或對於以上條款有任何疑問,Service@i-pass.com.tw或撥打客服專線(07)791-2000。

如因法令或情事變更而有更易必要,本公司有權隨時修改本告知書之內容,並將更新內容於網站上公告。


iPASS MONEY Notification of Personal Information Protection

Considering confidentiality of the collection of the Customer personal information, in respect to Paragraph 1 of Article 8 of the Personal Information Protection Act (the “Act”), iPASS Corporation (the “Company”) shall clearly inform the Customer regarding the following issues:

1. Purpose of collection
022 Foreign Exchange Business/ 040 Marketing/ 059 Financial service industry’s collection and processing information in accordance with laws and needs for financial supervision/ 060 Financial dispute resolution/ 063 Non-government agency collect or process personal information under legal obligations/ 067 Credit card, cash card, debit card or electronic value-stored card business/ 069 Contract, contract-like or other legal relation matters/ 090 Consumer, customer management and service/ 091 Consumer protection/ 148 Internet shopping and other electronic commerce services/ 157 Investigation, statistics and research analysis/ 181 Other business operation in accordance with the business registration project or organization Prospectus/ 182 Other Consulting and Consultant Services

2. Classification of the personal information collected
Name, address, telephone number, I.D. card number / ARC number, e-mail address, nationality, date of birth, account number applied for an online platform, records provided for network identity authentication or searching service application, number of a financial institution account, number of a credit card or debit card, serial number of a certificate, and other information detailed in relevant business applications or contracts, which depends on the personal data actually collected through business, accounts or services provided to the Customer, from the Customer, or from third parties (e.g., Joint Credit Information Center).

3. Period, area, parties, and way of using personal information
(1) Period: The longest period listed below shall prevail.
a. Within the time period of a specific purpose
b. The retention period as stipulated in relevant laws and regulations (The Act Governing Electronic Payment Institutions, etc.)
c. The retention period required for the execution of the business or the number of years of storage as specified in the individual contract
(2) Area: the location of the following parties of using personal information
(3) Parties: the Company, the organizations prescribed by law, the subcontracting agencies entrusted by the Company to deal with affairs, the institutions that cooperate with the Company (such as supervisory bank and cooperative bank) or the institutions that are engaged in business with the Company, competent authorities, financial supervisory authority and the parties with the consent of the Customer.
(4) Way: automatic machine or other non-automatic use in compliance with the relevant laws and regulations of personal information.

4. The rights and methods the Customer may exercise in terms of Article 3 of the Act
(1) The Customer may inquire and request a review or make duplications of the Customer’s personal information. However, the Company may charge the necessary handling fee in terms of Article 14 of the Act.
(2) The Customer may request supplementing or correcting the Customer’s personal information. However, according to Article 19 of the Enforcement Rules of the Act, the Customer shall give an appropriate statement of the reasons and facts for such request.
(3) In terms of Paragraph 4, Article 11 of the Act, the Customer may request the Company to discontinue the collection of the Customer’s personal information where a violation of this Act occurred during collecting, processing or using the Customer’s personal information.
(4) According to Paragraph 2, Article 11 of the Act, in the event of a dispute regarding the accuracy of personal information, the Customer may request the Company to discontinue processing or using its personal information. However, the preceding sentence may not be applicable when it is necessary for the performance of an official duty or fulfillment of a legal obligation and has been recorded, or when it is agreed by the Customer in writing.
(5) In terms of Paragraph 3, Article 11 of the Act, the Customer may request the Company to delete, discontinue processing or using the Customer’s personal information when the specific purpose no longer exists or time period expires. However, the preceding sentence may not be applicable when it is necessary for the performance of an official duty or fulfillment of a legal obligation, or when it is agreed by the Customer in writing.
(6) The Customer may make inquiry to the Company’s Customer Service Email (linepayipass@i-pass.com.tw) regarding the methods to exercise the aforementioned rights.

5. The impact on rights of the non-disclosure of personal information
The Customer may decide whether or not to provide the related personal information. However, the Company may not be able to proceed with the necessary business review or operation for providing the relevant service, or accepting the Customer’s user qualification and application for qualification of financial flows, if the Customer refuses to provide said information.

6. Consultation on the exercise of rights and matters of notification
If the Customer wishes to exercise the various rights as provided in the aforesaid Article 3 of the Act, or has any questions about the terms mentioned, please feel free to contract our Customer Service at linepayipass@i-pass.com.tw or at (02)6631-5190(07)791-2000.
If any modification is necessary due to changes in laws or circumstances, the Company reserves the right to modify the contents of this notification at any time and will make the announcement on the website.
